What We Offer

- Earn 60% of billable services with admin support

Key Responsibilities
- Assess care needs of patients
- Support families in implementing care plans
- Refer patients to appropriate care services and resources
- Provide short-term counseling and family mediation
- Offer practical assistance to reduce psychosocial stressors
- Coordinate discharge planning and aftercare support
Requirements
- Current and valid SACSSP registration
- Professional Indemnity Insurance
- Training in Palliative Care (required)
- Experience in Hospital Social Work and Case Management
- Knowledge in Oncology, Curatorship, or Frail Care is a plus
- Excellent report writing and time management skills
- Willingness to travel for outpatient sessions if needed
- Collaborative mindset within an interdisciplinary team
