What the Job Is About
As a Social Worker at Nurture Healthcare, you will play a vital role in the patient’s journey to recovery. You’ll work closely with therapists, nurses, doctors, and families to provide emotional and practical support for patients adjusting to rehabilitation.
Daily, you’ll support individuals who have experienced significant medical or physical trauma. Your job is to help them transition smoothly from the hospital to home or community care through coordinated care plans and counselling.
Your Key Responsibilities Include:
- Conducting full psychosocial assessments for new patients
- Developing tailored care plans for recovery and reintegration
- Offering counselling and crisis intervention for patients and families
- Facilitating discharge planning and family meetings
- Helping patients access essential resources, funding, and support
- Maintaining ethical, confidential, and accurate case records
- Collaborating with an interdisciplinary team to provide high-quality care

Skills & Qualifications Needed
- BA Degree in Social Work
