Nurture Health is looking for a dedicated and skilled Social Worker to join their team at the newly opened, 36-bed physical rehabilitation hospital in Montana, Gauteng. This exciting position offers the opportunity to work in a state-of-the-art facility that provides medical care and rehabilitation treatment for individuals with a wide range of disabilities caused by injuries or illnesses.
At Nurture Health, we specialize in treating people recovering from spinal cord injuries, traumatic brain injuries, strokes, neurological disorders, orthopaedic conditions, and other debilitating diseases. Our goal is to help individuals regain their independence and lead meaningful lives, no matter the disability they may face.
Key Responsibilities:
- Provide social work services to patients, ensuring their psychological well-being is maintained throughout the rehabilitation process.
- Collaborate with the multidisciplinary team to develop and implement care plans.
- Help patients and families adjust to changes in their condition and help them cope with challenges.
- Maintain accurate patient records and ensure all documentation is up to date.
- Act as a liaison between the hospital, patients, and community support resources.
- Support the rehabilitation process by assisting with discharge planning and community reintegration.
Requirements:
- A BA Social Work Degree and registration with the SACSSP (South African Council for Social Service Professions).
- Medical insurance is required.
- A minimum of 2-3 years’ experience in a private hospital environment.
- Experience in physical rehabilitation is highly advantageous.

Salary:
- The position offers a minimum salary of R26,464.20 per month (Cost to Company), which includes benefits.
